Welcome to Valley Parent!

This checklist is for families who are new to the school or who have had a one or more year(s) break from VPP. If you do not meet one of those conditions, then please visit to access the correct enrollment packet. For example, a returning family enrolling a new child would instead access the Returning Family, New Child packet.

This document outlines which paperwork you will need to submit to VPP as a part of the enrollment process for your family. For your convenience, we have gathered all of the forms a new or alumni family may need to submit and have included them below.  The paperwork will open in a new window - please be sure to save it to your computer.

Please keep a paper or digital copy of your forms for your records. VPP is not responsible for missing forms. Once complete, please print the entire packet and return it to VPP on or before May 31st.


Paperwork will be collected and reviewed on Wednesday, May 31st from 8am-10am at VPP. Children are welcome! You can also drop off or mail in your packets prior to that date. Incomplete packets will not be accepted—including packets that may only be missing one form or record—Registered families who do not submit complete paperwork packets by May 31st risk losing their registration spots.

Some of the forms may take longer to complete, particularly the adult immunization records (which will likely require a trip to your doctor’s office), so please plan accordingly. Thank you in advance for your cooperation in submitting the required information in a timely manner!
